In addition to this online form, we will also need any other unique paperwork for your student (such as an IEP, medical forms, or legal documents, etc). Those can be uploaded in the same section that you will upload Birth Certificate, Proof of address and Vaccination Records.

If your child lives outside of Alpine School District, or outside Westmore Boundaries, please contact Student Services at (801) 610-8518 to get the proper forms and information.

Student Services
Title 1 Preschool
Title 1 preschools serve three and four year old students who are identified as being at-risk academically. Students qualify through evaluations done at the school. All students must be three or four years old on or before September 1st of the school year they will attend, and MUST live within the boundaries of the participating Title 1 school.
CURRENT YEAR
If you are registering a preschooler for this current 2020-21 year, please check with the front office to ensure space is still available, and to schedule a testing appointment if there is still room.
NEXT YEAR
If you are registering for preschool next year, 2021-22, there are three easy steps to see if you qualify for Title 1!
- Fill out an application (you can fill out the online application, or come pick one up at the office!)
- Bring in copies of the birth certificate, immunizations, and proof of address. You can fax us the extra documents at 801-227-8744, or email scans to [email protected]
- Schedule a testing appointment! They will be held on May 10th, 11th, and 13th.
The preschool teacher can let you know if you’ve qualified after the test, and help set up the schedule for classes.
INCLUSION PRESCHOOL
We now have a second preschool option at Westmore for those who do not qualify for the Title 1 preschool! If you would like to register for the tuition-based, Inclusion Preschool run by the district but held here at Westmore, please visit the Inclusion Preschool registration form on their website.
